ABI Transitional - residential service

Residential Service Outline
Both Clifton Court and Ariya House are transitional/residential services for individuals with physical and /or cognitive needs.

The focus of the service is to maximise a person's independence and to develop their skills and confidence in a positive and progressive living environment.

Placements are designed around the individual's needs and a variety of options are available including:-

  • Short independent living or skills assessment placement
  • Medium length goal focussed transitional rehabilitation placements
  • Slow stream rehabilitation placement.

Where possible, we aim to skill individuals to progress onto more independent living.

The services are ideally located for access to both the town centres and local country parks. Both services have extensive links with community groups, vocational opportunities, events, classes and local facilities.

There are rail links connecting to Sheffield and the East coast main line to London and Edinburgh, and the trans-Pennine rail link to Manchester. Both services are located close to the M1 motorway.

Environment
Our ethos is to create an environment where people can thrive. It is important people are at ease, comfortable and feel valued. Therefore, so we take great care to ensure both the physical and interactive environment reflect this. We believe that people post-injury deserve and require a high standard environment to help they recover and adjust. The services are comprised of individual apartments with accessible kitchens and en suite wet-rooms finished to a high standard.

Ceiling track hoisting can be fitted as required. There is a physiotherapy room, a communal kitchen diner, a shared lounge, and a level access garden with raised beds.

We run on 'Prigitano inspired therapeutic community model' which means alongside personal rehabilitation programmes, we have peer support sessions. These have elements of sharing strategies and experience alongside skilled therapy interventions, to create an exceptionally supportive environment in which people can develop.

Therapy
Neuro-occupational therapy, neuro-physiotherapy, neuro-speech and language therapy, neuro-psychology, assisted technology and neuro-psychiatry.

Clifton Court and Ariya House have onsite neuro-OT and neuro-physiotherapy. The OT team provide dynamic outcome measured support, in both 1-1 mentoring session and skills groups. There are a variety of sessions, focussing nutrition, vocational rehab, cognitive strategies, technology support, life goal reflection work alongside independent life skills.

The physio team can provide onsite and offsite assessments, exercise and wellbeing classes and offsite hydro-physio assisted technology/IT expertise where required. Speech and language therapists work in conjunction the rest of the team to integrate plans into your lifestyle.

Neuro-psychology and Neuro-Psychiatry

We have integrated clinical neuro-psychology, which allows for full neuro-psychological assessment, 1-1 interventions, family support work and team training. Alongside the Neuro-Psychologists we have fulltime Assistant Psychologists in the services throughout the week provide you with responsive support. Our consultant Neuro -Psychiatrist works alongside the psychology team to support wellbeing of individuals and for response clinical medication review.

Our psychology team also support the families of those who have suffered an injury, both through 1-1 family intervention work or through our regular family support days.

All programmes are devised with the individual and integrated into functional daily living. Our team of proactive Rehabilitation Support Workers work alongside the therapy team to help support individuals achieve their therapy goals.

Therapeutic Environment
The service is also run as a person-centred therapeutic community, to encompass peer support work through psychology-lead, cognitive and self-awareness support groups. We run regular mindfulness support groups. Music/ Art and Drama therapy is available for those who may benefit.

The service is non-nursing, instead it is therapy led. We have a clinical lead nurse to monitor health related conditions and good links with the district nursing and community NHS teams. We use recognised outcome measures and integrate goals into functional daily living in line with the individual's aspirations.

Our emphasis is on community skill development, and where possible we work towards moving individuals onto more independent living.
